Summary Plan Descriptions
This section is designed to help you better understand the various benefits communication materials available to you as a plan member. The following documents, when combined for medical and life insurance plans, create a Summary Plan Description/Document.
Summary of Benefits Coverage (SBC) – An easy-to-understand summary about Hallmark’s medical plan benefits.
Benefits Guide – The benefit guide is intended to provide plan participants a detailed description of how the benefit plans operate.
Insurance Certificates – An in depth explanation of what benefits are covered under each Hallmark plan. The certificate describes the benefit level you should receive and how much coinsurance you will be responsible for paying. This document should be the first place you look to understand if a service or procedure is covered. It also will help you better understand appeal language regarding your benefits. Certificates are available for each benefit Hallmark provides.
